Quality of Life Health Services is Northeast and Central Alabama's largest primary health care provider with over 16 locations.  Services include adult medical care, pediatric care, dental, vision, diagnostic testing, lab, pharmacy, social services, women's health, enabling services, and behavioral health services.  

The clinic accepts Medicaid, Medicare and most private insurances. Uninsured patients may be eligible to receive 25% to 75% off services depending on their financial situation.  To receive reduced price care, patients must provide proof of income in one of the following forms:

• Check stub dated 6 months or less
• Food Stamp Letter
• Social Security Letter
• Medicaid or Medicare Card
• Unemployment Letter
• Current W-2 Form
• Bank statements
• VA
• Child Support

Patients must also provide proof of residency in one of the following forms:

• Mail addressed to patient
• Utility bills addressed to patient
