New Reports: Expired Drug Return Form, Ordering Reports, and Patient Contact List
In case you haven't checked out the "View Reports" screen in a while, there are three new reports. The first report is the "Expired Drug Return Form". If you would like to send back you expired medications, please print and complete the form and include it with your shipment back to the distribution center. Remember: If you are also donating medicines, be sure to separate your expired meds from your meds to be donated.

Now Accepting: Expired DOH Drug Returns!
By popular demand, we will now take back any DOH medications that have expired on your shelves. From this point forward, all orders will include an Expired Drug Return Form on which you will list each drug and quantity being returned, the serial numbers found on the yellow zip ties used to secure the bin, your site name, contact number, and signature. If you need additional bins, please call (888)428-HOPE(4673) and provide Kathy your dispensing site name and the number of bins needed. BE SURE TO INDICATE THE BINS ARE FOR YOUR EXPIRED DRUG RETURNS.
Cherokee Health System - Center City*
Cherokee Health Systems believes in a type of holistic care called Integrated Care in which mental health services are integrated into primary care.
The Cherokee Health Systems pharmacy is open to all Cherokee Health Systems patients. In addition to offering mail-order prescription services, they are the first pharmacy in Tennessee to help more patients in remote locations through the use of telepharmacy. Cherokee Health Systems accepts most insurance and TennCare plans and offers reduced cost care for those patients without insurance.
Methodist University Outpatient Pharmacy*
Methodist University Hospital has been a long-time partner with the Church Health Center and provides services for the Church Health Center's MEMPHIS Plan. The MEMPHIS Plan is not insurance, but is an employee-sponsored healthcare plan for small businesses and the self-employed. Benefits include primary and specialty care, hospitalization and other medical services. In 2010, the plan started to providing prescription services through the Methodist University Outpatient Pharmacy.
Dell Children's Medical Center, Outpatient Pharmacy*
Dell Children's Medical Center operates an outpatient pharmacy for patients discharged from the hospital and for patients seen at the affiliated pediatric clinics. Uninsured patients can receive prescriptions for free or minimal cost based on their household income.
